Young Innovators Workshop Participation Agreement, Parent Consent & Release

By registering my child for the Young Innovators Workshop at SK Technology Center, I acknowledge and agree to the following:

Parent/Guardian Consent: I certify that I am the parent or legal guardian of the participant being registered. I give permission for my child to participate in all activities associated with the Young Innovators Workshop.

Assumption of Risk: I understand that participation in hands-on educational activities involves inherent risks, including but not limited to minor injuries such as cuts, scrapes, bumps, falls, or other incidents that may occur during normal participation. I voluntarily assume all reasonable risks associated with my child’s participation in the program.

Medical Authorization: In the event of an emergency, I authorize SK Technology Center staff to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that I am responsible for any medical expenses incurred as a result of such treatment.

Health & Behavioral Expectations: I understand that participants are expected to: Follow instructor directions Treat other participants with respect Use equipment responsibly Maintain appropriate behavior in the learning environment Follow all safety guidelines provided by staff SK Technology Center reserves the right to dismiss a participant without refund if behavior repeatedly creates safety concerns or significantly disrupts the program.

Technology & Equipment Use: Participants may have access to computers, robotics equipment, maker tools, 3D printers, software applications, and other educational resources. I understand that reasonable care is expected when using all equipment and facilities.

Attendance, Cancellation & Refund Policy: Full Summer Passes and 4-Week Passes reserve a participant’s space in the program. Missed sessions are not guaranteed to be rescheduled or refunded. Daily Passes are valid only for the selected date. Refund requests submitted at least fourteen (14) days before the participant’s first scheduled session may be eligible for a refund, less any processing fees. No refunds will be issued after a participant’s first attended session except where required by law.

Photo & Media Release (Optional): YES, I grant permission for SK Technology Center to photograph or record my child during workshop activities and use such images or recordings for educational, promotional, marketing, website, social media, or program-related purposes.

Liability Release: To the fullest extent permitted by law, I release and hold harmless SK Technology Center, its owners, employees, instructors, volunteers, and affiliates from any claims, demands, damages, liabilities, costs, or expenses arising from my child’s participation in the Young Innovators Workshop, except in cases of gross negligence or willful misconduct.

By submitting this registration, I acknowledge that: I have read and understand this agreement. I voluntarily consent to my child’s participation. The information provided in this registration is accurate and complete. I agree to abide by all program policies and procedures.
